Commission Request Form

If interested in a commissioned costume or prop please fill in the form below. Please make sure to include what you would like to have made, the budget you are working with and the time frame in which you would need the commission completed by. If you have any special requests such as certain materials or color schemes please include those as well.

For all commissions I charge an hourly rate of $15/hr plus material costs. I require a deposit of 50% of the total cost of the commission up front before I begin the project which is accepted through PayPal or Venmo only. I am more than happy to work with you on pricing including payment plans ect.
